Can Certain Types of Physical Activity Improve Motor Skills in Children and Adolescents with ADHD?

ADHD is commonly treated with medication, but these treatments frequently cause side effects such as reduced appetite and disrupted sleep. Psychological and behavioral therapies exist as alternatives, but they tend to be expensive, hard to scale, and generally do little to address the motor difficulties that many children with ADHD experience — things like clumsy movement, poor handwriting, or difficulty with coordination. 

Physical exercise has attracted attention as a more accessible option. But research findings have been mixed, partly because studies vary so widely in how exercise is delivered and what outcomes they measure. This meta-analysis, drawing on 21 studies involving 850 children and adolescents aged 5–20 with a clinical ADHD diagnosis, tries to cut through that noise. 

Two types of motor skills 

The researchers separated motor skills into two broad categories: 

  • Gross motor skills — movements involving large muscle groups, such as running, jumping, throwing, and maintaining balance 
  • Fine motor skills — precise, controlled movements, typically of the hands and fingers, such as handwriting and manual dexterity (the ability to handle objects skillfully) 

The Data: 

Gross motor skills (16 studies, 613 participants) 

Overall, exercise produced medium-to-large improvements in gross motor skills. The strongest gains were in: 

  • Object control (e.g., throwing, kicking) — large improvement 
  • Locomotion (e.g., running, swimming), body coordination, and strength — medium improvements 

No significant gains were found in balance or flexibility. 

Fine motor skills (13 studies, 553 participants):

Exercise also produced medium-to-large improvements in fine motor skills, specifically: 

  • Handwriting: large improvement 
  • Manual dexterity: medium-to-large improvement 
  • Hand-eye coordination: moderate improvement

The Results: What Kind of Exercise Works Best? 

Two factors stood out consistently across both gross and fine motor skills: session length and frequency. 

  • Sessions longer than 45 minutes produced roughly twice the benefit of shorter sessions 
  • Three or more sessions per week outperformed less frequent programs for gross motor gains 

The type of exercise mattered; structured programs with clear motor-skill components (rather than unstructured physical activity) yielded stronger results. 

These results are not without caveats, however. The authors urge caution in interpreting these findings. A few key limitations include: 

  • Potential Publication Bias:  Studies showing positive results are more likely to be published, which can inflate apparent benefits. For gross motor skills, adjusting for this bias reduced the effect size from medium-to-large,  to medium. 
  • Active vs. Passive Controls: When exercise was compared against doing nothing (a passive control), improvements looked significant. When compared against regular school activities (an active control), the gains were no longer statistically significant. This is a meaningful distinction: it suggests exercise may be beneficial, but not dramatically more so than simply being physically active in a structured school setting. 
  • Medication status: Most participants were taking ADHD medication, so it’s unclear how well these findings apply to unmedicated children who might stand the most to benefit from structured exercise. 
  • Study quality: Many studies lacked proper randomization, weakening confidence in the conclusions. 

The Bottom Line 

This meta-analysis provides tentative moderate evidence that structured physical exercise can meaningfully support motor skill development in children and adolescents with ADHD — particularly when sessions run longer than 45 minutes and occur at least three times a week. The benefits appear most robust for object control, locomotion, handwriting, and manual dexterity. 

That said, the evidence base still has real gaps. The authors call for better-designed, fully randomized controlled trials with consistent methods, standardized ways of measuring exercise intensity, and greater inclusion of children and adolescents who are not on medication — all of which would help clarify when, how, and for whom exercise works best.

A team of Spanish researchers has published a systematic review of 16 studies with a total of 728 participants exploring the effects of physical exercise on children and adolescents with ADHD. Fourteen studies were judged to be of high quality, and two of medium quality.

Seven studies looked at the acute effects of exercise on eight to twelve-year-old youths with ADHD. Acute means that the effects were measured immediately after periods of exercise lasting up to 30 minutes. Five studies used treadmills and two used stationary bicycles, for periods of five to 30 minutes. Three studies "showed a significant increase in the speed of reaction and precision of response after an intervention of 20-30 min, but at moderate intensity (50-75%)." Another study, however, found no improvement in mathematical problem-solving after 25 minutes using a stationary bicycle at low (40-50%) or moderate intensity (65-75%). The three others found improvements in executive functioning, planning, and organization in children after 20- to 30-minute exercise sessions.

Nine studies examined longer-term effects, following regular exercise over many weeks. One reported that twenty consecutive weekly yoga sessions improved attention. Another found that moderate to vigorous physical activity (MVPA) led to improved behavior beginning in the third week, and improved motor, emotional and attentional control, by the end of five weeks. A third study reported that eight weeks of starting the school day with 30 minutes of physical activity led to improvement in Connor's ADHD scores, oppositional scores, and response inhibition. Another study found that twelve weeks of aerobic activity led to declines in bad mood and inattention. Yet another reported that thrice-weekly 45-minute sessions of MVPA over ten weeks improved not only muscle strength and motor skills, but also attention, response inhibition, and information processing.

Two seventy-minute table tennis per week over twelve weeks improved executive functioning and planning, in addition to locomotor and object control skills.

Two studies found a significant increase in brain activity. One involved two hour-long sessions of rowing per week for eight weeks, the other three 90-minute land-based sessions per week for six weeks. Both studies measured higher activation of the right frontal and right temporal lobes in children, and lower theta/alpha ratios in male adolescents.

All 16 studies found positive effects on cognition. Five of the nine longer-term studies found positive effects on behavior. No study found any negative effects. The authors of the review concluded that physical activity "improves executive functions, increases attention, contributes to greater planning capacity and processing speed and working memory, improves the behavior of students with ADHD in the learning context, and consequently improves academic performance." Although the data are limited by a lack of appropriate controls, they suggest that, in addition to the well-known positive effects of physical activity, one may expect to see improvements in ADHD symptoms and associated features, especially for periods of sustained exercise.

Meta-analysis Finds Young People with ADHD Are More Likely to be Lonely Than Non-ADHD Peers

A research team based in London (U.K.) has just released the first meta-analysis to explore whether young people with ADHD experience greater loneliness compared to young people without ADHD.  

There has been a lot of research into social-emotional functioning in ADHD focusing on social networks and peer functioning problems, with little inquiry into young people’s experience of and satisfaction with their relationships.  

The team noted, “This is an important distinction as loneliness, or perceived social isolation, is the subjective feeling of distress due to a perceived deficit in the quantity and quality of one’s social relationships. An individual may be objectively socially isolated but may not have a negative perception of their relationships and equally, someone may find their social relationships lacking despite having a large social network … As loneliness and social isolation (e.g., number of friends or size of social network) have been found to be weakly correlated, it is important to capture the loneliness experience of the young person rather than relying on just the objective measures of social isolation.” 

Loneliness is a public health concern, as it predicts increases in depression, anxiety, self-harm, and suicidal ideation. 

The team performed a systematic search of the peer-reviewed medical literature to identify quantitative studies that compared loneliness among young people under 25 diagnosed with ADHD with non-ADHD matched controls. The search excluded studies in which participants had intellectual disabilities and/or other neurodevelopmental conditions such as autism spectrum disorder. 

Meta-analysis of 15 studies with a combined 6,281 participants found that young people with ADHD experienced greater loneliness than their non-ADHD peers with a small-to-medium effect size. There was no sign of publication bias, but very high variation (heterogeneity) in outcomes across the studies.  

Removing one small outlier study, the remaining 14 studies with 6,099 participants brought heterogeneity down to moderate levels, and yielded a medium effect size increase in perceived loneliness among young people with ADHD.  

Conclusion: 

“The findings in this study highlight the importance of understanding loneliness in this population as young people with ADHD report significantly higher levels of loneliness compared to their non-ADHD peers, and loneliness in young people with ADHD is associated with a range of mental health difficulties,” the researchers stated. “More resources should be focused on loneliness as a separate construct from social isolation and peer difficulties.”

Meta-analyses Suggest Physical Exercise is Effective Tool in Treating ADHD

Two meta-analyses suggest physical exercise is an effective tool in treating ADHD

Two recent meta-analyses, one by an Asian team, and the other by a European team, have reported encouraging results on the efficacy of physical exercise in treating ADHD among children and adolescents.

One, a Hong Kong-based team (Liang et al. 2021) looked at the effect of exercise on executive functioning.

The team identified fifteen studies with a combined total, of 493 participants that met the criteria for inclusion. As the authors noted, "only a few studies successfully blinded participants and therapists, due to the challenges associated with executing double-blind procedures in non-pharmacological studies."

After adjusting for publication bias, the meta-analysis of the fifteen studies found a large improvement in overall executive functioning.

The studies varied in which aspects of executive functioning were addressed. A meta-analysis of a subset of eleven studies encompassing 406 participants found a large improvement in inhibitory control. A meta-analysis of another subset, of eight studies with a total of 311 participants, found a large improvement in cognitive flexibility. Finally, a meta-analysis of a subset of five studies encompassing 198 participants found a small-to-medium improvement in working memory.

Nine studies involved acute (singular) exercise interventions lasting 5 to 30 minutes, while twelve studies involved chronic (regular) exercise interventions ranging from 6 to 12 weeks, with a total duration of 12 to 75 hours. The chronic exercise was more than twice as effective as acute exercise. The former resulted in large improvements in overall executive functioning, the latter in small-to-medium improvements.

No significant differences were found between aerobic exercises (such as running and swimming) and cognitively engaging exercises(such as table tennis and other ball games, and exergaming ... video games that are also a form of exercise, relying on technology that tracks body movements).

The authors concluded that "Chronic sessions of exercise interventions with moderate intensity should be incorporated as a treatment for children with ADHD to promote executive functions."

Meanwhile, a German study team (Seiffer et al. 2021) looked at the effects of regular, moderate-to-vigorous physical activity on ADHD symptoms in children and adolescents.

They found eleven studies meeting their criteria, with a combined total of 448 participants. A meta-analysis of all eleven studies found a small-to-moderate decline in ADHD symptoms. However, the three studies with blinded outcome assessors found a large and statistically highly significant decline in symptoms, whereas the eight studies with blinded outcome evaluators found only a small decline that was not statistically significant.

When compared with active controls using pharmacotherapy in a subgroup of two studies with 146 participants, pharmacotherapy held a small-to-moderate advantage that fell just short of statistical significance, most likely because of the relatively small sample size.

The authors concluded that moderate to vigorous physical activity (MVPA) "could serve as an alternative treatment for ADHD," but that additional randomized controlled trials "are necessary to increase the understanding of the effect regarding frequency, intensity, type of MVPA interventions, and differential effects on age groups."

The Neurocognitive Roots of Boredom in ADHD: a Meta-Analysis

Boredom is more than just feeling restless or under-stimulated. It’s a negative emotional state that arises when activities feel meaningless or dull and, for those with ADHD, this negative emotional state might be markedly more intense. Researchers increasingly view boredom as functional: an internal signal pushing people to seek more rewarding and meaningful experiences. But for some, that signal becomes chronic and overwhelming.

People who are highly prone to boredom face a range of psychological and behavioral consequences, including anxiety, depression, difficulty identifying their own emotions (alexithymia), impulsivity, and physical complaints. These struggles often surface in harmful behaviors: overeating, substance use, compulsive internet use, and gambling.

For people with ADHD, boredom can cross into genuine distress. Many describe it as “torture” or “an itchy coat you can’t scratch”,  language that conveys not mild discomfort but an urgent, almost unbearable need to escape. This makes sense given that ADHD involves core difficulties with attention, arousal regulation, and motivation, all of which make sustained engagement harder and boredom far more likely.

The Study:

A recent meta-analysis of 18 studies involving more than 22,000 participants confirmed a moderately strong and consistent positive association (an overall effect size of r = 0.40) between ADHD and self-reported boredom. All but one study found significant results, and there was no evidence of publication bias.

“While the relationship between ADHD and boredom may seem obvious,” the authors state, “this has paradoxically led to the phenomenon being understudied.”

Despite how significant this connection appears to be, the researchers noted it has attracted surprisingly little scientific attention; a gap they attribute to a widespread assumption that boredom in ADHD is simply a byproduct of inattention or impulsivity, and therefore not worth studying on its own terms. They push back on that view, arguing that boredom may be a more fundamental part of the ADHD experience: a bridge between atypical brain function and the behavioral, emotional, and cognitive difficulties that shape long-term outcomes.

The Take-Away: 

Ultimately, addressing the profound boredom experienced by individuals with ADHD requires a multifaceted approach that goes beyond simply treating inattention. Researchers emphasize the need for rigorous studies to determine if stimulant medications actively reduce this intense boredom by repairing underlying brain mechanisms, rather than just as a side effect of improved focus. Beyond medication, tailored psychological therapies may offer promise; psychoeducation can help individuals reframe boredom as a biological signal rather than a personal failure or character flaw. 

Additionally, another approach suggests that rather than solely focusing on treating the individual, systemic issues must be addressed, such as the effects of low-stimulation environments. For example, prioritizing a better "person-environment fit" through smaller class sizes, flexible academic pacing, and/or offering highly stimulating, novel tasks, schools and workplaces can offer meaningful relief from the chronic distress of ADHD-related boredom.

Early Skull Fusion in Infants Linked to Higher ADHD Risk

A new study from Japan suggests that infants born with craniosynostosis are significantly more likely to be diagnosed with ADHD later in childhood. Craniosynostosis is a condition in which the bony plates of the skull fuse prematurely, leading to increased intracranial pressure. 

The Background:

Craniosynostosis affects roughly one in every 2,000 births. When the skull’s natural seams close prematurely, it can restrict brain growth and increase intracranial pressure, potentially reducing blood flow to the brain. Because the condition is relatively rare, it has been difficult to study at scale until now. 

The Study:

To overcome this, researchers tapped into a large Japanese insurance database compiled by JMDC, Inc., which holds records on around 20 million people, or about 15% of Japan’s population. Drawing on two decades of data, the team tracked over 338,000 mother-child pairs. Children with related genetic syndromes or chromosomal conditions such as Down syndrome were excluded to keep the focus on craniosynostosis itself. 

Of the children studied, around 1,145 had craniosynostosis, and 7,325 were diagnosed with ADHD. After accounting for factors like sex, birth year, maternal age, mental health history, pregnancy infections, and birth complications, children with craniosynostosis were found to have roughly 2.4 times the risk of a subsequent ADHD diagnosis compared to those without it. 

To test whether shared family genetics or home environment might be driving the association rather than the skull condition itself, the researchers conducted a separate analysis among siblings. The elevated risk remained at 2.2 times. The consistency of the finding across both analyses strengthens the case for a genuine biological link. 

The Results:

The results point to raised intracranial pressure and restricted cerebral blood flow as plausible mechanisms, though the study’s observational design means causation cannot be confirmed. Ultimately, these findings highlight the need for proactive, long-term care strategies for those born with craniosynostosis. By establishing a solid link between premature skull fusion and a significantly higher risk of ADHD, the research demonstrates that medical care for this condition should not end once the skull's physical structure is addressed.

The Takeaway:

Pediatricians, neurologists, and parents can use this data to implement early, routine behavioral and developmental screening for these children as they grow. This additional support would ensure that those who do develop ADHD can receive timely interventions, educational aids, and therapies, ultimately improving their long-term developmental outcomes.

Population Study Indicates ADHD Drug Treatment May Reduce Contact with Child Welfare Services

Children and adolescents with ADHD come into contact with child welfare services (CWS) far more often than their peers. There are many contributing factors to consider, including the fact that hyperactivity and impulsivity frequently lead to behaviors that are considered disruptive and cause academic and social difficulties. Many of these children are also growing up in households marked by parental conflict and/or single-parent arrangements.  All of these circumstances can compound vulnerability and, historically, increase the likelihood of CWS involvement.

Background: 

In Norway, Child Welfare Services operate at the municipal level and are legally required in every local authority. Their scope spans investigation, family support, and, where necessary, out-of-home placement and ongoing monitoring. Grounds for intervention include abuse, neglect, behavioral or psychosocial difficulties, and inadequate care-giving. Norwegian CWS works closely with health, education, and social services and places a strong emphasis on keeping families together. Compared with systems in countries such as the United States, Poland, Romania, and the Czech Republic, the Norwegian approach sets a lower bar for intervention and leans toward home-based support, while setting a higher bar for out-of-home placements. This model is shared by other Nordic countries, as well as Germany and the United Kingdom. 

Research into whether ADHD medication affects child welfare caseloads is remarkably sparse. A single Danish study previously found that medication treatment accounted for much of an observed decline in foster care cases, but no study had examined medication’s broader impact on CWS involvement, covering both supportive interventions and out-of-home placements. 

Norway’s universal single-payer health system and comprehensive national registers make population-wide research of this kind feasible. Drawing on these resources, a Norwegian research team set out to test whether ADHD medication reduces children’s contact with CWS and their need for out-of-home placement. 

The Study:

This study included all 5,930 children and adolescents aged 5 to 14 who received a clinical ADHD diagnosis from Child and Adolescent Mental Health Services between 2009 and 2011. Each was followed for up to 4 years post-diagnosis, the upper age limit being 18, at which point CWS jurisdiction ends. This group was compared with more than 53,000 peers who had no CWS contact during the same period. 

The results showed a meaningful, though not dramatic, association between medication and reduced CWS contact. At one year, treated children had approximately 7% fewer contacts with CWS; by two years, that figure had risen to around 12%. The effect then narrowed, settling at roughly 7–8% reductions at the three- and four-year marks. 

The picture for out-of-home placements is considerably less convincing. The research team highlighted a 3% reduction at two-year follow-up, but this finding barely crossed the threshold of statistical significance, and no effect was observed at the one-, three-, or four-year follow-up points. 

The Take-Away:

The authors concluded that pharmacological treatment for ADHD is associated with reductions in both supportive CWS services and out-of-home placements among children affected by clinicians’ prescribing decisions in Norway. A more cautious reading of the same data, however, would emphasize an overall reduction in CWS contact of roughly 8%, while treating the out-of-home placement finding as, at best, inconclusive.
